To create a custom pre-discovery call questionnaire that collects essential client information before the first consultation.
STEPS
1. Navigate to Forms
From the dashboard, go to Web Pages
Click Forms
Review available templates (including the Discovery Call template)
Proceed to create a new custom form
2. Create a New Form
Click Add New Form
Choose to start from scratch
Keep the default fields already included in the form
3. Add a Checkbox Field (Symptoms)
Drag a Checkbox field into the form
Edit the Label with your question (e.g. symptoms experienced)
Set a clear Query Key
In Advanced Settings, set the Unique Key
⚠️ Note: The Unique Key cannot be changed after saving
Add your checkbox Options
Important Note on Custom Fields
Once created, a custom field can be reused in other forms
Editing field options later will update that field across all forms where it is used
4. Add a Radio Button Field (Duration)
Drag a Radio Select field into the form
Configure the label, query key, and unique key as before
5. Add Date of Birth Field
Locate Object Fields
Under Contact, drag the Date of Birth field into the form
No additional configuration is required
6. Add a Multi-Line Text Field (Open Description)
Drag a Multi-Line Text field into the form
Set the Query Key and Unique Key
7. Save the Form
Click Save
Return to the dashboard
Verification
Form saves successfully
All fields appear correctly

If options change across forms, confirm whether the same custom field is reused
